Depends mostly on the controller used, but you generally continuously keep looking either up or down, and you try to make left / right moves that lasts for only one frame. More than 1, and the game re-position you to the boundaries, so it's really that tight.

For consistency and hand-stamina, I'd suggest not spamming the buttons. About the precision part, it maintly comes from personnal experience, and everyone has their own methods of doing it.

As far as I know though, there only are a very limited amount of places you can do it, and only one of them is useful for anything (That's for the newer 6-1 skip).

I am sorry for not being able to help improve yourself better, I am not that much knowledgeable about this one game in terms of speedrunning. There isn't a lot of runners for this game to begin with, and almost all of them are inactive here.
