Doing these will force you to reset the console, probably killing the run. Will update this if I find more softlock situations (feel free to post any others you find too)

• The Pits (Swaghouse) - When freeing the dreamflight as Hannah, mashing triangle too much causes her to die then respawn unresponsive. To avoid, either don't mash triangle, or stop mashing before the teleportation sequence starts.
• Swaghouse clock - Using the clock items too quickly one after another will prevent the animation from playing properly and you become permanently stuck. To avoid, wait a brief moment after the animation before using the next clock.
