So start with pistol only, no upgrades used, no bought armor/ammo. Thug or Gangsta difficulties only. Seem fair?

I agree with Wagner. It just running to the end of the level and killing the boss. Also who wants to see IL's on easy mode with upgraded ammo or health from the start?

A low percentage can be interesting but not enough to warrant a category in my opinion. Unless you play on the higher difficulties only.

I did that in earlier runs a few times. You place your head right along the upper ledge and lightly tap left + b while on the ladder and the ball should by thrown next to or close to the ladder. Kinda tricky and sometimes it don't work right.

The whole jumping on ladder is new to me. Can you ride up without any climbing?

I mean you can add that category but don't think anyone would run it. I sure wouldn't. It doesn't add that much value.

I was tinkiering on Hard mode today and I notice it shows the secret path veeeeery slow. Finn which emulator are you using? bgb might not be optimal for this game.
