Binding to 1 didn't work because only the F-keys and ~ (tilde) work on the main menu. The 'map' command didn't work in-game because you were already in a session, so the 'changelevel' command should be used instead.

You need to hold crouch, not just press it.

This ostensibly still happens on the first load if blue has srconfigs, as the 'developer 1' setting from singleplayer is still set, which skips the cutscene.
