I think ng is where you don't have any power thingies and you don't choose your character

Once you finish a full run of 6+ days, you will get access on the loading screen to choose 'New Game +', so I think you may need a full regular run completed to get access to it. NG is (I'm assuming), day 1 new game started fresh.
