To get the good ending you need to have banked 500 points or more after the first boss fight. You score points by killing an enemy. Base enemies are 1 point, skeletons and knights are 2, beard boys are 3, bird is 5, 1st final boss is 10. You get a 2x multiplier if they die via cliff and a 3x multiplier if they die via magic. Each level also has its own multiplier, however, each death you take in a level drops that multiplier to the next tier like this:

If you keep dying, the multiplier drops for points accrued in that level. I assume it bottoms out somewhere, but I didn't experiment with that.
I understand this map skips some levels, but I think the others slot in logically here as levels 2-6

I would run an All 27 Team category tomorrow if there was such a thing. In fact, I thought I did see it at some point, but now it's gone? Maybe I dreamed it.

If you check the "Resources" tab, there is a download that lists the load removal. You just manually do it when you submit a run.
The link didn't work for me. However, this isn't in the NHL series. It is in the NHL 2K series. I see now that the 2K series are mixed in with the NHL series for some reason. That seems incorrect to me.
