You start from the beginning of an applicable chapter. The run ends when you have gotten to a piano and triggered every piano easter egg. Only chapters 4 & 5 will have this for now, but i presume at least chapter 6 will have it. It's a meme run, but it does have the skill of playing the piano and memorizing the songs you need to play.
