I think that we could have Misc, categories for completing each Chapter, and then maybe, just maybe, Level Leaderboards for this game. They could be for each of the 28 levels (maybe include the secret levels too). These are just ideas to make the leaderboard bigger and hopefully make the game a bit more competitive :D

It could also help runners of this game keep track of their times per level and see how much improvement they're making

A leaderboard with more categories makes the game ¤¤less¤¤ competitive (which is more competitive, 10 people on one category or 1 person on each of 10 different categories?) This game is small enough that there really doesn't need to be a lot of misc categories that are run by one person. If more people provide competition, the leaderboard could track them, of course.

I am not opposed to adding level leaderboards, however. Does the game provide an IGT for individual levels? That would be my preference.

The only levels that have an IGT are the Road and Ship levels (ex. Firetruck, Ambulance, Cleanup, Aircraft), and iirc, every other level does not include an IGT, and there are only about 7 levels that fall under the category of Road and Ship levels

To be completely honest, the only category extensions I'd really be on board with would be Medium difficulty (which would probably be boring AF since it'll just be the annoying parts of Hard and Easy combined) and a category involving playing all the extra missions. At the end of the game, it tells you that there are like "extra missions" for each level that you play by hitting Z or whatever on it. So a category where you play the full story and then those could work, but then I don't even think those are that different to even make it worth doing.

Also to my knowledge there is no IGT timer. There is one on some of the vehicle stages, but then those times can be adjusted by getting the hourglass power up.

Long story short, I feel like we have about as many categories tracked as would be possible. There are more possibilities of course, but I highly doubt many more people are gonna run them. I appreciate some of the enthusiasm and ideas, but I just don't see it getting much deeper unfortunately.
