Are you running DX12 or DX11?

did you do the OoB strat after Jack 1? and did you use the newest route for Madhouse? bc if you did, you skip over the trigger that despawns Jack in the main house just before you reach Jack 2. so Jack is still in the Main house because you never hit the trigger. He despawns if you keep progressing through the game
