I accidentally discovered a glitch where the item changes. If you already have an item, you need a mushroom and another item, and you have to collect them in the same frame. I think it is only in multiplayer possible. Perhaps someone can find out if it's also possible in single-player mode.
it looks like it tho. the fireflower that should've been used or put back into the item slot changes into a bell on pick up. Same with the bell that turns into boomerang
yea he started with cat + fire and ended with 2 cats, kept cat equiped while saving cat at the same time
that's a cool exchange glitch
