100% run - are we referring to just 100% of items & map... or also 100% of Orbs as well?

I think we should discuss this with others. I feel all orbs and items should be considered, the map in my opinion not really but it does count it after the credits so that we can decide to leave it or not.

100% should be all items, map completion, and orbs as shown on the file selection screen.

It should be whatever gives the file the 100% upon completion

Items, Orbs, and Map are tracked - fun fact, Keys gathered DO NOT count towards 100% items (worth noting), neither does whatever the hell that door in Amethyst Passage is about.
