Is there a known way that the trigger for the Wilhelm boss fight doesn't happen?

Got there in my very first run today and simply nothing happened, no boss, no train carriage rising up, no dialogue, nothing.

Along side this the infinite ammo stopped working and wouldn't work on multiple attempts of getting it back up again.

Is this a known thing that can happen or is the answer here that I'm just bad and have no idea what I'm doing?
