Wasn't sure where to post this so I'm putting it here. I played through the game trying to collect all gems to get a count on the gem total in the game. Here's my list:

That brings me to the total MachineDJP got in his walkthrough of the game. His total was: S386 E383 R381 D386
That means I missed some gems he got but I also came out with 2 more Sapphires than him. So I don't think we can one hundred percent confirm the total of gems in the game for the 100% category.
If I think MachineDJP did not do everything and you too, you may have forgotten gems in the water in the sewers in the labyrinth of the First Kingdom of Fire
After re looks all the corners that you think you have not passed like in the rather narrow place or even isolated corners. I think there are gems that you have forgotten in the realm of death.
As we say we have no choice, we must make every ounce of the game XD
I might replay through the whole game following along with MachineDJP. Getting everything he gets.
I did that for the Death Realm. Just followed along with him so if I missed something there than he missed it too.
I also eventually want to make diagrams of all the levels that show all collectible locations. I already made a few to help me with routing 80%.

I got in contact with Stephen Clarke-Willson who was the producer & director of Azurik. He sent me a map from 2001 that tells the total number of gems in each area. http://imgur.com/a/elJ5J
That brings the new total gem count to: S387 E386 R382 D386 = 1541 (That's 5 more gems that MachineDJP missed)
I'm still not one hundred percent sure that this is the final total though because in my playthrough I got S388. So I'm wondering if I'm wrong or something on the map is wrong. Also on the map there are no totals for the Life and Death Realms. So I can only assume I found all the ones in those areas.
