300% + All S and P Ranks + All Achivements (if your copy of the game was achivement-less, use a achivement-ingame mod, unless if it's banned) + Get All Effect Modes (this include 2S, BaW, Chalice and Vintage).

Add Category for Every Character, 100%, Any Levels except tutorial 100%, Tutorial.
