Is there a question in here somewhere? Are you looking for help with fixing the controller? It's obviously defective/broken if it's randomly inputting things different from what you're doing; it's certainly not expected behavior. It could be caused by any number of things - the inside could be dirty, the inside could have a piece loose, or there could be some other issue with it. It's hard to say without physically opening the controller and trying to examine it.
