Mechanics for Level Progression, Difficulty, & Scoring

Some comments on runs of the SNES game hinted at people not fully understanding how scoring and such work until relatively recently (even though most of the relevant info can be recovered from the manuals).

In the interest of making this available to people, I decided to compile all of that information for the SNES & GB releases in a Google Doc, and propose ideal settings for runs of each category based upon that.
