There should be a 100% category, which includes the Double Combine quest and the RAGE token/Raging Demon quest.

There should also be subcategories for the three difficulties: Casual, Normal, and Hardcore.

I'd be happy to create a 100% category if you have a video of the run.

As for difficulty sub-categories, the only reason I didn't add them initially was because when I played on Casual, the maximum health was capped to 100 like in Normal (until you die, then it resets to 150). For that reason, I assumed that something about the difficulty selection was glitched.

Looking at your runs, this doesn't seem to be the case, and I don't know why. I've tried the 1.1 update, but still had the glitch happen for me, and this isn't mentioned in the changelog.

I don't think this is much of an issue however, just thought I would mention it. Everything else aside from max health appears to work as intended on Casual (no knockback, no depleting rage, etc.), so I'll be adding those difficulty sub-categories now.

Hey Walgrey, I know what's up with the glitched health in Casual difficulty. For whatever reason, if you immediately play a Casual difficulty speedrun when you boot up/reset the game, it doesn't set the health to 150 though every other aspect of Casual is there (like no knockback). However, I found that if you go into the Time Attack or Boss Attack modes and complete one on Casual, it will correctly set the health back to 150 when you start a speedrun.

I don't know why this is a thing but it's something I picked up on after my first finished run. It only means that you'll have to set up a Casual difficulty run if you want to run that, other difficulties seem to work correctly upon reset.
