that's because you have the keys. It will only spawn if you didn't get the Keys upgrade. It's intentional and probably the hardest part of the run imo

The requirements for 100% are all powerups, as the other collectables don't have an in-game counter to measure. These are: All buckets: you finish with the blood bucket. All weapons: you finish with the golden broom (that includes finding all dust piles) All spells: you finish with the splash spell

These are acquired from bosses or mini-bosses, except for the golden broom that I will let you discover (hint: it's in the white area)
