Pretty sure that the newest version has some extra cutscenes and bug fixes, and the sewers section is easier. Might not be everything as I never bothered to play it myself. The older version is faster.
Sorry, I didn't see this until now. The latest version of the game has some additional cutscenes in the sewers level. This is great for casual play since it makes the objectives a bit more clear for a first time player. For speedrunning, those cutscenes lose some time. I forget how much exactly but its not a lot, maybe 10 seconds or less? Its still faster to play on the old version however.
