Is there a difference in the pacifist run if I just play it in regular, or do I have to unlock expert first?

Run and gun levels are exactly the same regardless of whether Expert is unlocked.

The only difference (which is irrelevant for Pacifist runs) is that the maximum non-pacifist score becomes an S instead of A+ after you unlock Expert even though the level remains the same.
