You have to pick up the red hand, not just let your hand sit on it, the trigger is at the door of the red hand room and extends to about halfway through the second prison cell

You are probably picking up the hand to late, the trigger for their spawning doesn't cover the whole survival section
consoles are the only versions with 1.15+
Pc is on 14, but there are no differences (I think) between 1.13/1.14 on base game so its just 1.13 on the leaderboards. Ruin is what is affected by the changes in 1.13 to 1.14
