Notes on Shaman Showdown (U): Hold K and press Q to unlock (almost) all levels. F4 = Fullscreen F5 = Save state (:O) F6 = Load state Chosen language affects enemy chat messages, lol. Portal Offensive has no IGT clock. Portals for Pros has an anvil in the first area.

I can't really tell if there are any other gameplay changes, but I'll keep playing 1.02 to be consistent.

You're right, I just got this old copy (~2011) and it unlocked with M+Z. Other than that my save didn't work so the only other difference I saw was the language select. Very weird.

Apparently there's another secret level called Portal Offensive, though it's not clear on how to unlock it...

(Incidentally this video has some speedy clips, Frostbite is surprisingly similar to mine even though I only found it today.)
