i think you were on the wrong side of the dragon, making it harder to aim

no, the dragon's head was pointing to the obsidian, and not on the bedrock so I want to know what causes it :/

I wish It did not happen on a top 5 run :(

It has to do with changing the render distance in the end and making the dragon load or some shenanigans with that, so to prevent this from happening change the render distance before the end.
