Well, the record is currently 2:22:47 as of last night. I was pretty tired watching Seth stream so I didn't see the seconds/milliseconds. Biggest issue was that he kept dying to Calamity, and despite practicing Thunderblight a lot, the fight wholly dependent on patterns since you can only damage Thunderblight with a flurry rush.
On the whole it looks like taking a few minutes to grab/cook more food would really help out. Just looking at Orcastraw's 3:39:39 run, you might be able to grab stamina/speed shrooms on the plateau, and there has to be some food on the way to the castle that's worth detouring for. Between that, checking for another quickly accessible Royal Guard's Bow, and just not dying in the middle of the boss rush, it should definitely be possible.
