I dont see the problem with 3 difficulties. Some games have more than that.
If anything, standard makes less sense than assisted (at least for Leon at this point, cause of Claire's running speed in caution). Assisted for max speed, Hardcore for the challenge.

Agreed with Trichael Man. No reason not to have it. Stuff like "Aim assist and health regen" is not a reason to not allow it for New Game.
There shouldn't be a real difference between Xbox and Xbox 360. So you are free to run it on w/e platform you prefer. Most people run it on 360 tho I would assume.
The run is based on real time. Ingame time is actually shorter but it's kinda weird. Like it's off a lot from the actual time. I recommend putting the ingame time in your comment when you submit your run. Just to be save.
A common mistake that many people make is right at the beginning of the fight. When you shoot the first Ganado you have to make sure you have the correct distance to him. Look at some shadows on the ground, they should mark like a big X. Thats a good visual sign on where to stand. If you are too close to the guy, the next wave will spawn in a different spot. So you will kill less enemies with your grenades. Only thing I can think of why it doesn't work for you.
Only works with the normal and special costume 1. Make sure Ashley is in the follow mode. Can't have any weapon equipped, it has to say empty. Aim the knife at Ashley, while doing so also press down the button you would normaly aim your weapon with. Keep holding down the weapon button and release the knife button. Jump down, still holding down the weapon button. Run over to the switch and hit it. You can now release the weapon button which makes Ashley follow you again. Make sure you stand at a spot where the enemies can't directly see you.

Sure! The Steam version is the fastest version of the game. 60fps is faster than 30 fps~
The game does have an autosplitter. Open livesplit > edit splits > chose Resident Evil 4 as the game > activate the autosplitter underneath.

I would recommend to just run the PC steam version. It has the fastest loading times out of all versions and with that the fastest possible cleartimes. It tends to crash tho and suffers from some performance problems.
If you dont care too much about the speed you could always go for the WIi or the HD Console versions. Very stable versions, but slower than steam. NTSC Gamecube is quite different from other versions for speedruns, so I would not recommend it.
Yes, the ditman glitch works on the PS4 like on any other version. Most NG+ runs are done on normal difficulty, so yes, you can do that. As long as the ingame time is visible at the end of the game, it will be fine.
