I was watching his VOD and his time is a 3:47/3:48. He does the final split a couple of seconds after he get's bitten by the lady. So he deserves the best time spot :)

Thank you for pointing this out. I did my run back when the only other speedrun was by Progamingwithed (http://www.twitch.tv/progamingwithed/c/6201944) so I used his timing method of splitting when the heart appears in the end. I would like to ask the mods to specify the rules for this game so we can have a uniform start and ending. I would also be happy to help out with the moderation of this page if any help is needed.
