I tested all of the fps dependent jumps in the game, and recorded the lowest fps I was able to complete the jump with. Anything fps dependent that isn't listed only requires 100fps or lower, so I didn't feel the need to put them in.

The only other puzzles I see this exploit working in are the A Star puzzles, mainly Nerve Wrecker, and to optimize floor 5 in the tower.
Starting at the first user input sounds good to me. People were starting at new game and first initial input, so I thought we should all start at the same time.
I agree with banning ff, or making a separate category if people want to run it. For measuring time, I think it should be real time since looking at a backup doesn't give you the actual time, like Grokken said. And a set start should be made (starting the time when you press new game).
