Mmm, if I'm not wrong the blueberry pie is only used if going for the secret ending, so the only diference from the normal run would be having to do all 4 levels on the first hub, all 3 Humanibli levels and the 4th hub. Of these three things, the only one that might pose a problem is the humanibli levels, as they would require reentering levels to enter them and fighting Humanibli 3 times in the same run each time, and he can get ¤reeeeally¤ tanky, specially on the first one without Void Block.

Personally, I'm waiting for Ryan to release the steam version with the extra boss to see what are the requirements to fight him and what he changed about the game, though it seems from his twitter that it will take a while for it to come out. Though otherwise I think doing a 100% category is a great idea and I might restart doing runs for it!
