There's a task in chapter 1 which requires you to repair two fountains. There's a glitch which causes the task to complete after repairing only one. Here's how to consistently perform it:

1. Be on a save which has already completed that task
2. Exit out of it
3. Start a new save

When you reach the fountain section it should complete after repairing the first one.

I'd imagine this happens because some flags aren't reset when you exit out of a save or start a new character. It's possible there are similar skips later on in the game but we won't know unless someone runs in to them.
