I know fast melee was banned but what exactly was it?
Was it buying the perk blitz? Meleeing zombies while crouched? Or just simply having the shovel out was considered fast?
I would just like some clarification on the ruling and what it all included so that I and my friends avoid using it in future runs.
it was the crouch melee that allowed you to attack several times a second.

Unless i was told wrong crouching and melee is something that all platforms can do and is allowed, however there is a fast melee glitch that we banned that can only be done on the PC version of the game. To do this glitch you would shoot and hold down the shooting button (LMB) and then you could spam the melee button as fast as you could.
