Weapon swap off of knife and back onto it really quickly, while attacking is my understanding of the glitch

It's not a glitch, you switch weapons back and forth really quickly for knife. Careful not to fire the gun that invalidates the whole run.

yes, I've done it. It is hard to do though (did this back on the demo)

You have to switch to the pistol then back to your knife quickly. You have to literally press both buttons almost at the same time after landing the knife attack. It's much tougher to do on controller since you probably need to move your hands in uncomfortable positions to get it done. KB/M is perfect for this trick.

I've been able to get it to work on og Xbox one and the d-pad on Xbox controllers is not good. Personally I need practice but it is doable

Can you test that at 120 FPS? I see that you did that at 60 FPS, but I believe it may be easier to do at lower FPS since the trick may be almost a frame perfect input. The higher the frames, the harder it is to pull off.

No I don't spam. When you spam you lose control of jill. Except last one try that I wanted to see how many consecutive hits could I get.
It's all about the timing and 'feeling' of the buttons. Feel comfortable with your controller and try to put both of your thumbs at each of the shortcut. Hope I helped!

I am averaging 2-3 with my controller lately. Never broke 3 though. Been doing this on console for a few months now. lol
