If you didn't already record it there's nothing you can do about it, just record it with your phone or something in the future (quality doesn't need to be the best) so it doesn't happen again

You don't. There's fields for it on the run submission form. Put your video (or.......screenshot WutFace ) where you'd put video or screenshots (YouTube, imgur, etc). You link to it here. You don't actually upload it here
