(This is the document I used while making my initial 100% route. It's terrible, but I'll leave it here in case anyone wants a starting point for the 100% route.)
RESET-POINT = Able to reset on this level to get 3 stars. NO-RESET = No Reset. Cannot gather 3 stars in-level, so previous RESET-POINT is necessary. X-POSSIBLE = Not enough boxes in this level, but can 3-star. M-POSSIBLE/NO-RESET = Did not kill all monsters, but can 3-star anyway. G-POSSIBLE/NO-RESET = Did not gather all gems, but can 3-star anyway. Q-POSSIBLE/NO-RESET = Did not kill all enemies or gather all gems, but can 3-star anyway.

Level 1 - RESET-POINT Notes: 2 boxes. No stone-only enemies.
Level 2 - RESET-POINT Notes: 3 boxes. 2 fish.
Level 3 - RESET-POINT Notes: 2 boxes. No stone-only enemies.
Level 4 - NO-RESET Notes: 1 box. 2 fish.
Level 5 - RESET-POINT Notes: 2 boxes. 2 fish.
Level 6 - X-POSSIBLE Notes: No boxes. No stone-only enemies.
Level 7 - RESET-POINT Notes: 2 boxes. 1 porcupine.
Level 8 - X-POSSIBLE Notes: 1 box. No stone-only enemies. Spike ball forces a hit if you have power-ups.
Level 9 - M-POSSIBLE Notes: 1 box. 1 porcupine. Ignoring porcupine gives 3 stars.
Level 10 - NO-RESET Notes: 1 box. 1 porcupine. 1 wasp cannot be jumped upon. Porcupine cannot be safely passed with helmet without stones.
Level 11 - NO-RESET Notes: 1 box. 2 fish; 10 porcupines. Do not break vertical stairs to gem. Must reset at Lv. 7 to 100%.
Level 12 - NO-RESET Notes: 1 box. 4 fish; 7 porcupines. Can reset from lv. 11.
Level 13 - RESET-POINT Notes: 2 boxes. No stone-only enemies.
Level 14 - NO-RESET Notes: No boxes. 5 porcupines.
Level 15 - X-POSSIBLE Notes: 1 box. No stone-only enemies.
Level 16 - M-POSSIBLE Notes: 1 box. 1 porcupine. You can skip 1 porcupine and get 3 stars.
Level 17 - NO-RESET Notes: 1 box. 4 porcupines.
Level 18 - NO-RESET Notes: 1 box. 3 porcupines; 4 fish.
Level 19 - NO-RESET Notes: 1 box. 3 porcupines. Watch out for wasps when climbing vertically.
Level 20 - RESET-POINT Notes: 2 boxes. 7 porcupines, 4 fish. Painfully slow to sneak past the porcupines for the boxes.
Level 21 - NO-RESET Notes: 1 box. 1 porcupine.
Level 22 - X-POSSIBLE Notes: No boxes. No stone-only enemies. The cycles for the swinging spikes should work.
Level 23 - NO-RESET Notes: No boxes. 1 porcupine, 1 fish. Must reset from lv. 20 to 100%.
Level 24 - X-POSSIBLE Notes: No boxes. No stone-only enemies.
Level 25 - RESET-POINT Notes: 2 boxes. 13 fish. Resetting here may be slower than a future point, given the waiting for the fish.
Level 26 - RESET-POINT Notes: 2 boxes. 1 porcupine. Vertical level. Watch out for wasps from above.
Level 27 - M-POSSIBLE Notes: 1 box. 2 porcupines. Will give 100% if you miss 2 porcupines.
Level 28 - M-POSSIBLE Notes: No boxes. 1 porcupine. Will give 100% if you miss 1 porcupine.
Level 29 - X-POSSIBLE Notes: 1 box. No stone-only enemies.
Level 30 - NO-RESET Notes: No boxes. 5 fish. Possible to reset from Lv. 27 to get stones required.

Level 1 - RESET-POINT Notes: 2 boxes. 3 fish.
Level 2 - NO-RESET Notes: 1 box. 1 grasshopper, 2 porcupines.
Level 3 - RESET-POINT Notes: 2 boxes. No stone-only e nemies.
Level 4 - RESET-POINT Notes: 2 boxes. 7 fish; 4 mosquitoes; 1 grasshopper, 1 porcupine. One of the mosquitoes is above the boxes. Keep mind of spike ball cycle with crabs beneath.
Level 5 - NO-RESET Notes: 1 box. 2 porcupines. 1 wasp hugs the ceiling and cannot be jumped on.
Level 6 - NO-RESET Notes: No boxes. 4 mosquitoes. Watch out for the spiked balls.
Level 7 - RESET-POINT Notes: 2 boxes. 2 fish. Do not break any boxes with a helmet! :O
Level 8 - NO-RESET Notes: 1 box. 9 fish; 2 porcupines. Several wasps at ceiling.
Level 9 - RESET-POINT Notes: 2 boxes. 5 fish.
Level 10 - M-POSSIBLE Notes: No boxes. 1 Grasshopper. Do not hold right at start of level. Can complete level and miss 1 Grasshopper.
Level 11 - M-NO-RESET Notes: 2 boxes. 2 Porcupines; 10? Wasps; 12? Crabs. You can miss 1 Wasp. 3 Wasps appear at the lower platform with the square of Gems @ 50 seconds remaining.
Level 12 - NO-RESET Notes: 1 box. 5 mosquitoes. Vertical level.
Level 13 - NO-RESET Notes: 1 box. 9 porcupines. 3 wasps at start heading left.
Level 14 - NO-RESET Notes: 1 box. 5 fish. Spiked ball mid-level.
Level 15 - RESET-POINT Notes: 2 boxes. 12 fish; 3 porcupines. Watch out for running out of time.
Level 16 - NO-RESET Notes: 1 box. 5 fish; 1 mosquito. Several spiked balls that are off-screen. :O
Level 17 - NO-RESET Notes: 1 box. 4 mosquitoes.
Level 18 - RESET-POINT Notes: 3 boxes. 13 fish.
Level 19 - RESET-POINT Notes: 2 boxes. 5 porcupines; 2 mosquitoes; 1 grasshopper. Watch out for running out of time!
Level 20 - NO-RESET Notes: No boxes. 18 fish. Pixel perfect jump at the start.
Level 21 - NO-RESET Notes: 1 box. 4 fish; 2 porcupines. Be patient with the wasps at the start and the end.
Level 22 - NO-RESET Notes: 1 box. 5 mosquitoes; 3 porcupines; 2 grasshoppers.
Level 23 - G-NO-RESET Notes: 1 box. 1 Porcupine; 3 Fish. Also, 1 Wasp in an irritating place. Returning to Level 21 possible. Possible to get 3 stars and miss 1 gem.
Level 24 - NO-RESET Notes: 1 box. 4 fish; 3 porcupines. Watch out for wasps while platforming near spikes over water.
Level 25 - G-NO-RESET Notes: 1 box. 16 fish; 2 porcupines; 2 grasshoppers; 2 mosquitoes. Possible to get 3 stars and miss 1 gem.
Level 26 - RESET-POINT Notes: 3 boxes. 1 porcupine; 9 fish. Tight jump @ 1st fish.
Level 27 - G-NO-RESET Notes: 1 box. 2 porcupines; 1 Mosquito. Must damage boost through spikes w/ helmet to gain gems. Possible to get 3 stars and miss 1 gem.
Level 28 - NO-RESET Notes: 1 box. 5 fish. Watch out for wasps near water.
Level 29 - M-NO-RESET Notes: 1 box. 8 porcupines. Possible to get 3 stars and miss 1 porcupine.
Level 30 - NO-RESET Notes: 1 box. 12 fish; 3 porcupines. Pixel perfect jump @ box. May be easier to take out crabs before gathering upper path gems.
