First follow the instructions written under "Manual Installation" at the bottom of the page.

then:

• press add and put 523578 into 'base'
• name of process is POP2 (steam) or pop2 (GOG)
• left text is something like StoryCounter or whatever you want
• and right text should be {0} wich is the id of the memory adress
